WAIVER OF LIABILITY

It is recommended that participants consult with a physician prior to the start of any physical activity. By registering for or participating in a City of Barrie program, which includes attending a drop-in recreation program, or partaking in any activity in any City of Barrie facility, the participant or Parent/Guardian of the Participant(s) agree they will be voluntarily participating in these activities and the participant(s) assumes all risks of injury or death, which might result from these physical exercises or activities.
I, the Participant or Parent/Guardian of the Participant(s) warrants and represents that the participant is in an appropriate physical condition for these activities and that the participant has no disability or impairments preventing the participant from engaging in these activities, or that will be detrimental or injurious to the participant's health, safety and physical condition or that of others. I, on behalf of myself and the Participant(s), other heirs, next of kin, executors and administrators hereby release, waive and forever discharge any claims against the Corporation of the City of Barrie, its employees, officers, directors and agents from all claims arising from any accident, death or injury or property damage which is caused by or arises from participation in any City of Barrie program or use of City facilities.
I, the Participant or Parent/Guardian of the Participant(s) shall also comply with all public health and safety guidelines, provincial or federal regulations, statutes and all other public or government requirements with respect to Covid-19 (“Covid-19 Requirements”) and shall be solely responsible for any liability whatsoever arising as a result of failing to adhere to any Covid-19 Requirements and hereby agrees to indemnify Barrie from any claim whatsoever associated with such failure.
MEDICAL AUTHORIZATION
I hereby give permission and consent, on behalf of myself and the above-named person(s) for whom I am in law responsible (the Participant(s)), to have City staff administer or arrange for any emergency medical care including hospitalization/transportation if necessary, and to the administration of such emergency medical treatments as may be deemed necessary in the circumstances. All participants are responsible for their own medical coverage.
I confirm that I have had sufficient time to read and understand each item of this Waiver and Consent in its entirety, and agree to be bound by its terms freely and voluntarily.
Withdrawals and Refunds
Withdrawals and requests for full refunds will be considered if received prior to the 2nd day of the program. 5 business days notice is required for Camps and Aquatic Leadership courses in order to be considered for a full refund. In all cases, a $15 administration fee will be charged (per person, per receipt). Refunds may be prorated based on the remaining classes. The effective date of withdrawal is the date the withdrawal notice is received by the City, regardless of the date the participant stopped attending the class. Withdrawals and Transfers can be done online at play.barrie.ca in accordance with our Refund Policy timelines. To withdraw/transfer on or after the first day of the program, call 705-739-4223, emailing recreation.information@barrie.ca or in person at one of our main community centres.

COLLECTION OF PERSONAL INFORMATION

Personal information on this form is collected under the authority of the Municipal Act, R.S.O 1990, c. M.56 and will be used for enrollment into/or purchase of City of Barrie Recreation & Culture Services products and services.
