FOIP NOTIFICATION STATEMENT 34(2)

The personal information that you provide to Mount Royal University is collected under the authority of the Post-Secondary Learning Act and the Alberta Freedom of Information and Protection of Privacy (FOIP) Act - Section 33(c). The information will be used for the purpose of providing you various recreational and educational services through the Mount Royal University Recreation Department, which includes membership management, facility bookings, and course/program enrolment. Mount Royal University has contracted ActiveNet to provide recreation participant management software in order to administer membership services for Students, Faculty, Employees, and external Clients. All Users are advised that any information stored with ActiveNet may be housed on servers residing in another province and, therefore, may be subject to the laws of another jurisdiction outside of Alberta. Although Mount Royal University has made every effort to protect the privacy of Users, the University cannot guarantee protection against possible disclosure of information residing in another jurisdiction. Collected personal information is protected from unauthorized access, collection, use, and disclosure in accordance with the FOIP Act and can be reviewed upon request subject to the provisions under the Act. GENERAL INFORMATION FOR ACTIVITIES

Some activities have withdrawal policies that supersede the general refund policy (see activity description for more details.)

Up to seven (7) days prior to the activity start date:

Refunds, less a 10% administration fee, will be processed. Transfers from one activity to another, subject to availability, will be processed with no administration fee.

From six (6) days up to 48 hours prior to the activity start date:

Refunds, less a 10% administration fee, will be processed. Transfers from one activity to another, subject to availability, will be processed with a 10% administration fee.

Within 48 hours prior to the activity start date:

No refunds will be provided. No activity transfers from one activity to another.

After activity start date:

Prorated refunds, less a 10% administration fee, will only be processed with approval from the program supervisor for medical reasons. Note: this does not apply to certification and leadership courses.

Cancellations:

Recreation reserves the right to cancel activities due to low enrolment, inclement weather or environmental hazards. Note: some activities run rain or shine.

Non-members can gain entry to Recreation 15 minutes prior to the scheduled start time of the activity. If you require more time, various membership package options are available by purchase. Note: this does not apply prior to MRU Recreation's scheduled opening time.

Recurring Credit/Debit Card Payment and Storage Statement

If you have chosen to enter into an agreement to save your credit card for future use, or for use in a payment plan, please note the following: credit card details are stored in truncated format (ie: last four digits of PAN). You will be notified 90 days in advance of any changes to this consent via either (a) the email you supplied when you created your account or (b) the @mtroyal.ca email credentials associated to your student/employee account. The stored credit card credentials will only be used to process the payment plan amounts indicated on this receipt.

ACKNOWLEDGMENT OF RISK

There is inherent risk with participation in any recreation activity. As a voluntary user of Mount Royal University Recreation Centre's facilities and equipment (collectively the "Premises") and / or a participant in the Mount Royal University Recreation Centre's programs (the "Programs"), you knowingly assume any and all risks associated with physical activity or your use of the Premises or participation in the the Programs. You assume responsibility for ensuring that you understand how to properly and safely use the Premises and are medically and physically capable of performing physical activity during your use of the Premises or participation in the Programs. You assume responsibility for all risks associated with your participation. Risks may include, but are not limited to:

-minor injuries such as scratches, bruises, strains and sprains;

-major injuries such as eye injury or loss of sight, joint or back injuries, cardiac events and concussions.

-catastrophic injuries including paralysis and even death.
