By registering as a participant of any online or in-person programming offered by The Corporation of the City of Sarnia, its subsidiary and affiliated entities (“The City of Sarnia”), I agree to the following:

I will be participating in wellness and recreational activities including, but not limited to beginner-friendly exercise classes, seminars, meditation, conversation groups and arts programming (collectively, the “Activities”) offered by The City of Sarnia. During the COVID-19 pandemic, the “Activities” normally offered in the physical location of the Centre may be offered online by videos, television, podcasts, apps or other digital media or platforms. All of such offerings, either physical or online, shall be considered “Activities.”

I understand that the Activities offered by The City of Sarnia may be physically strenuous and I participate voluntarily in the Activities. In consideration of my participation, I hereby acknowledge that I am aware of the risks, dangers and hazards associated with or related to the Activities which could result in damage, loss, serious physical injury, or death. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in any of the Activities. I further agree that the sole responsibility for my (and/or child’s) safety remains with me, and I assume all risks arising out of, associated with or related to my (and/or child’s) participation. I hereby waive any and all claims that I (and/or child) may have now or in the future against The City of Sarnia for any personal injury, property damage/loss, wrongful death, whether caused by negligence or otherwise.

In further consideration of my (and/or child's) participation in this activity, I hereby release and forever discharge The City of Sarnia, its local boards, subsidiary and affiliated entities, its elected officials, directors, officers, employees, agents, volunteers, independent contractors, representatives, and any successor and assigned (hereinafter referred to as “Releasee”) from any action, cause of action of any nature and kind whatsoever for injury or, as the case may be, damage sustained or that may be sustained as a result of my (and/or child’s) participation in the Activities. I (we) agree to follow the rules and regulations set by The City of Sarnia and above named parties.

Furthermore, for the said consideration, I hereby covenant and agree to indemnify the Releasee from and against any liability, damages, costs and expenses suffered or incurred by them as a result of any claim made or proceeding taken against the Releasee arising from my (and/or child’s) participation in the Activities.

I do hereby grant and give The City of Sarnia the right to use my or my child(s) photograph or image with or without my or my child's name, both singly and in conjunction with other persons or objects, in presentations, advertising, publicity, and promotion relating thereto.

A $12.00 administrative fee will be applied to any refunds, unless deemed unnecessary by staff. If notice of cancellation is received within the designated days, a credit to account less the refund fee will be processed. If notice of cancellation is received on the day of or after the program date, no refunds will be issued. Please see the full cancellation policy at www.sarnia.ca.

Please note that we are not able to give partial refunds for missed classes unless for medical situations, doctor's note will be required. Payment for course registrations are due upon booking, and if you cannot attend the program it is your responsibility to contact our office to cancel before the program starts. A $12.00 administration fee for refunds will apply.

I have read the above release waiver of liability and fully understand its contents. I voluntarily agree to its contents. I voluntarily agree to the terms and conditions stated above.
