Understand care in the community, hospital discharge planning, public and private home care, and residential care! Join educator, public speaker, independent healthcare navigator, and patient advocate, Connie Jorsvik, in this workshop designed for patients and families. Since 2011, Jorsvik and her team have supported hundreds of patients and families through illnesses, end of life, and future planning. Registration is required.

In order to get the care we need we must be informed, empowered and assertive. This workshop focuses on understanding care in hospital, discharge Planning and a focus on understanding both private and public subsidized home care services and residential care.
