Excellent Ice program, I acknowledge that participation in athletics carries with it a risk of physical injury. I agree that Excellent Ice it’s agents and employees shall not be liable to me or my child for any injury or damage, howsoever caused, resulting directly or indirectly from child’s participation in any and all Excellent Ice programming at any time preceding, during or after program is in session and I hereby discharge Excellent Ice, it’s agents and employees from all actions, claims, and demands I or my child may have for any such injury or damage. I also understand that Excellent Ice is given the irrevocable right to use my name/photograph/image/ video recording/ and likeness (“My Image”) in all forms and manner including but not limited to publication on Internet Web Sites, Social Media, and any other publications as released to or by Excellent Ice. I understand that it is my responsibility to cancel my registration if I do not agree to the terms and conditions as set out above by Excellent Ice.
