Privacy and Confidentiality
Most sessions will be implemented in a group format, and will include discussions based on insights/personal experiences. You and other participants have the right to confidentiality and privacy by the group leaders and other group members. Niagara Region’s Seniors Community Programs adheres to professional, legal and ethical guidelines of confidentiality. By attending this group you agree not to share any other participant’s information outside of the group.
Guest speakers and students may be involved in some sessions to allow for specific content by a professional or enhanced experiential learning for both students and clients.
Online programs:
Communication via online, email or Zoom is not without risks, especially concerning confidentiality. Information about confidentiality and privacy while using the Zoom platform can be found at zoom.us.
Consent for Release of Information
I understand that for the purpose of offering its services to myself, Seniors Community Programs will collect, use and may disclose my personal health information when needed.
Any information collected is kept confidential unless Ontario or Canadian laws require disclosure.
All Personal Information collected, used and disclosed will be in accordance with the Municipal Freedom of Information and Protection of Privacy Act (MFIPPA) and the information will be used only for the purposes of administering Seniors Community Programs.
